Create and Connect Your Support Form
Sign into your Jotform account.
Design a new custom form from scratch with our drag-and-drop Form Builder, or select one of our 10,000+ templates.
In the Form Builder, click Settings in the toolbar, and then select Integrations from the Settings sidebar.
Select the Zendesk app, enter your Zendesk subdomain, and click Authenticate. You will then be prompted to log into your Zendesk account.
Once your Zendesk account has been authenticated, you can match your form fields to your Zendesk fields.
The form fields Name, Email, Subject, and Description are required to generate a support ticket.
Map additional form fields to custom ticket fields, select a group, and add any tags.
If a form contains an upload field, this file can be attached directly to the support ticket.
Click the Save button to complete the integration.
That’s it! When your form receives a new submission, it will automatically be sent to Zendesk as a new support ticket.

Enhance Your Integration with AI
If you’re using Jotform AI Agents, you can take your Zendesk integration further:
Sign into your Jotform account.
You can either build your agent from scratch or choose a ready-made template from our AI Agent Templates gallery to get started faster.
In the AI Agent Builder, click Train in the toolbar and select Knowledge Base from the Train sidebar.
Select Zendesk, enter your Zendesk subdomain, and authenticate your account.
In the Trigger section, define custom prompts or keywords that should lead to ticket creation. For example: “I need help,” “Something’s broken,” or “Follow up on my issue.”
Configure ticket details, like subject, description, tags, or assignee, to match how your Zendesk team handles tickets.
Enable the Create Zendesk Ticket action and click Publish to activate your AI Agent.
Your AI Agent is now ready to monitor conversations and automatically create Zendesk tickets based on user input.
